Today, we operate within an industry of over 20 competitors in our service area, predominantly for-profit entities. As a non-profit, we prioritize our people over profits. We are unique in our dedicated team, state-of-the-art hospice facility, and extensive outreach efforts.

Furthermore, we take immense pride in being the home of the Wayne T. Patrick Hospice House, the ONLY hospice house in our area. This unique facility provides a level of care and comfort that is unparalleled in our community. Additionally, we extend support to those in our care and our community through charity care for the underinsured, a Caring Closet to lend medical supplies, and grief counseling available to all in need. None of these vital services would be possible without your support!

    Support Hospice & Community Care by making a gift today. Your donation, whether one-time or recurring, helps us provide compassionate care to those in need. You can give in honor or memory of a loved one, and we will send a personalized letter to the family notifying them of your thoughtful contribution.   • Sanctuary Garden Pavestones

    The Sanctuary Garden features and beautifully landscaped seating areas which offer an opportunity for peaceful contemplation. Woven throughout the garden are hundreds of pavestones that create a lasting monument to loved ones who have influenced others’ lives. Pavestones come in three sizes: 9 X 9, 6 X 9, and 6 X 6.

  • Veterans Garden Pavestones

    The Veterans’ Garden honors and memorializes the continued importance Veterans represent in our lives and our hearts. Veterans’ Garden pavestones come in two sizes, or you can choose to have a brick engraved and placed in one of the service branch ribbon paths. The sizes available for the Veterans’ Garden pavestones include: 6 X 9 and 6 X 6.

    The Hospice & Community Care campus and the Wayne T. Patrick Hospice House were made possible through generous donations from local individuals, families, and businesses. For a more permanent legacy, we are pleased to offer a variety of naming opportunities.

    Naming offers a sense of personalizing the memorial both for those lost and the surviving loved ones. A range of naming opportunities are available for individuals, families, organizations, and corporations, enabling donors to pay tribute to their relationships with Hospice or to honor and recognize someone special in their lives.
