Hospice & Community Care is a non-profit, mission-driven organization seeking a full-time qualified volunteer coordinator. The Volunteer Coordinator is responsible for the recruitment, screening, orientation, training and coordination of all volunteers and for the administration and development of the volunteer program in the agency’s service area. Responsibilities include assessing the needs of the patients and families and providing support services as needed. Candidates must have excellent interpersonal skills and the ability to speak in public.
Hospice & Community Care is a non-profit, mission-driven organization seeking a full-time Qualified Nurse Practitioner to provide Hospice & Palliative Care Services for patients in facility and home care settings. Candidate will work with Board Certified Physicians in an independent work environment. Experience in Hospice, Palliative and/or Long Term Care setting preferred. Multi-State/SC License and CPR required. Must be flexible and possess excellent communication and interpersonal skills.
Registered Nurses (RNs):
RN will be assessing, planning, implementing, and evaluating patients, primarily on weekends. This position will assist the other team members in coordinating services for the patient in accordance with the physician and will also need to maintain communication regarding patient care needs. Makes visits to patients to provide nursing care, instruction and support to patients and families (i.e. pain and/or symptom management, crisis intervention, death visits, admissions). Presents services, obtains initial physician orders, and completes all necessary paperwork related to nursing admissions and other visits as necessary. Must have CPR and have licensure in state or be eligible for licensure in state.
A minimum of 3-5 years experience in healthcare, home care or hospice setting is preferred. Positions also require excellent communication and interpersonal skills as well as reliable transportation.
